Hydrogen Ion Concentration and Acid-base Equilibrium in Normal Pregnancy

چکیده
In 1912 Hasselbalch, with Lundsgaard and Gammeltoft (6, 7), described a lowering of the alveolar carbon dioxide and of the carbon dioxide-combining power of the blood in the later months of normal pregnancy. These changes have been abundantly confirmed by others and found to appear early in the course of pregnancy. These findings together with an increased ammonia to total nitrogen ratio in the urine described by Hasselbalch and Gammeltoft (6) have long been considered as an expression of an increased acid production in pregnancy. This conception has arisen partly from the impression that Hasselbalch and his associates detected a reduction in the pH of the blood in pregnancy. In actual point of fact they did not determine the pH of the blood directly, but only after exposure to a known concentration of COZ in air. This is equivalent, of course, to measuring the bicarbonate content or alkaline reserve and yields no information concerning the actual reaction of the blood in the body. The distinction was clearly recognized by Hasselbalch who concluded that the hydrion concentration of the blood in pregnancy remained normal. Hasselbalch did, however, interpret the increased ammonia to nitrogen ratio as an indication of an abnormally great acid production. It is now recognized that acid production is proportional not to the urinary ammonia to nitrogen ratio, but to the ab-

The Acid-base Equilibrium in Abnormal Pregnancy
In previous papers (9, 14) we have discussed the changes that occur in the acid-base equilibrium of serum of normal women during pregnancy. In brief, these changes are reduction of the serum carbon dioxide content and serum proteins, accompanied by a comparable reduction of total base. These findings have been confirmed by Stander, Eastman, Harrison, and Cadden (21). The reduction of serum prot...

Balance studies have been carried out to evaluate the influence of vasopressin-induced volume expansion on acid-base equilibrium in normal dogs and in dogs with steady-state metabolic acidosis induced by the administration of 5-7 mmoles/kg per day of hydrochloric acid.Hypotonic expansion in dogs with metabolic acidosis (mean plasma bicarbonate concentration 14 mEq/liter) produced a marked incre...
متن کاملThe effects of chronic hypoxemia on electrolyte and acid-base equilibrium: an examination of normocapneic hypoxemia and of the influence of hypoxemia on the adaptation to chronic hypercapnia.
We have carried out balance studies in normal dogs in order to appraise the effects of chronic hypoxemia on acid-base and electrolyte equilibrium. During the first phase of observation we produced a state of "pure" hypoxemia by reducing the oxygen concentration (utilizing nitrogen as a diluent) and by adding carbon dioxide to the environment in a concentration sufficient to keep arterial CO(2) ...
متن کاملThe Carbonic Acid-carbonate Equilibrium and Other Weak Acids in Sea Water
Sea water is a salt solution of about 3.5 per cent concentration whose various constituents are maintained in almost constant proportions. A notable exception to these constant relations occurs in the case of carbon dioxide, which is found to vary greatly in concentration. These facts of variation arc undoubtedly related to the conditions of organic activity, and the concentration of carbonic a...

PHYSIOLOGICAL AND CHEMICAL CONSIDERATIONS Definitions: Van Slyke72 has defined acidosis as a condition in which the bicarbonate of the blood or serum, or, more exactly, the base available for combination with bicarbonate, is diminished. Alkalosis is a condition in which there is in the blood or serum an abnormal excess of bicarbonate, or base available for combination with bicarbonate. These de...
